How to register

Patients living in the M41 postcode wishing to register with the Practice should contact the reception staff who will give them the required forms to complete.

You can register by completing a form available from reception. To save time you can also download and print a registration form

You will also need to complete one of the following new patient questionnaire forms:

New Registrations questionnaire 5 & under

New Registrations questionnaire 10 & Under

New Registrations questionnaire 11+

You will need to complete these by hand and bring into the surgery.

Please note that you are registering with the practice and not a particular GP, however if patients express a preference for a particular doctor we will endeavour to help. Please bring details of any medication you are currently taking with you to your appointment.

We recommend that new patients undertake a health check with a practice nurse.

Temporary patients

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days.

After 14 days you will need to register as a temporary or permanent patient (see Temporary Patient Registration Form). You can be registered as a temporary patient for up to three months. After three months you will have to re-register as a temporary patient or permanently register with that practice.
